• Second line choice for the treatment of formally diagnosed scabies, established clinically and/or by parasitological examination. Use restricted to patients who have had treatment failure with topical treatments.
• First line in exceptional cases for patients with challenging behaviours where effective topical treatment is not possible.
• Consider oral ivermectin over permethrin in patients who have pre-existing eczema or other skin conditions which may lead to hypersensitivity.

• Pruritus/suspected contact (unlicensed indication). Ivermectin is only licensed for treatment of diagnosed scabies.

• Crusted (Norwegian) scabies, on advice of secondary care dermatology.
• Other indications, on advice of microbiology.
